KORT is Kentucky's premier provider of outpatient physical and occupational therapy specializing in orthopedics, sports and industrial medicine. Since 1987, KORT has provided clinical and customer service excellence to patients in our clinics, at employer sites, and with high school, college, and professional sports teams.
In addition, KORT Home Health also provides home health services in Southern Indiana including: post-operative physical therapy; fall prevention; vestibular rehabilitation; wound care; and more.
The Nurse Case Manager delivers professional nursing care for client in accordance with the established plan of care and agency policies and procedures. Provides care consistent with all relevant professional standards and state and federal requirements. Demonstrates proficient skills using Oasis to admit, transfer, re-certify, and discharge home health patients in regards to physician orders. Coordinates and supervises LVN’ s and HHA’s in the delivery of patient care. Must maintain compliance with agency policy and procedures and follow Medicare and Medicaid regulations.
Must be a registered nurse (R.N.) with a current Indiana license.
Minimum of one year of nursing experience as a R.N.
One (1) year of Home Health experience preferred.
Work positively and favorably with patients, families, and staff.
Strong verbal and written communications skills.
Flexible with diverse client assignments.
Current automobile liability insurance and current unrestricted drivers license.
CPR certification is mandatory immediately after employment.
