Case Manager - PRN - Inpatient Rehab

Baylor chose to enter into this partnership because the core competency of BHCS was the medical aspect of the business. We wanted to be the best in Rehabilitation Services and the joint venture helped us continue on the vision to offer top notch quality care, provide more service lines to our patient and to be in the top five hospitals in the nation. We have an opening for a PRN Case Manager.
The Case Manager manages the discharge process of patients from the inpatient facility as well as the Day Neuro Program at the same facility. Facilitates interdisciplinary team meetings and family conferences. Coordinates continuum of care.
