Responsibilities

Choose to be an integral part of a team that helps patients achieve their greatest potential and work toward productive, independent lives. The Registered Nurse (RN) This position exists to provide specialized nursing care for patients in the outpatient setting. Uses the nursing process for systematically assessing, planning, implementing and evaluating patient care to maximize restoration and maintenance of the patient’s physical, psychological, spiritual health and goals.  Serves as an educator of patients, families and co-workers.

 

We're looking for an experienced RN with a BSN for our Baylor Institute for Rehabilitation Physician Outpatient Clinic. Experience in OP, Inpatient rehab or med/surg/Telemetry/Acute Care is preferred. 

Employees in this position will:

  1. Utilizes the nursing process of assessing, planning, implementing and evaluating patient care with a priority on the education of the patient/family/significant other based on the health care needs identified. Uses a positive and encouraging approach to achieve patient goals and quality outcomes.
  2. Utilizes critical thinking to prioritize and manage complex patient care. Identifies and clarifies patient needs, focuses on patient safety and the quality of nursing care by applying appropriate nursing interventions.
  3. Identifies professional nursing care needs for patient populations in the ambulatory rehab setting. Identifies teaching needs and goals related to the diagnosis of each patient and collaborates with patient/family and the interdisciplinary team.
  4. Serves as liaison between patients and health care providers to provide efficient follow up care. Facilitates continuity of care using multidisciplinary collaboration, and coordination of appropriate health care services and community resources. Assists the patient to navigate the health care system and assure clinical outcomes that are appropriate, cost-effective and beneficial to the patient.
  5. Develops and maintains updated departmental community resources, materials and patient education information. Responsible for keeping abreast of current information regarding entitlement programs patients may need to utilize.
  6. Schedules and prioritizes workflow and patient care while managing multiple priorities. Manages patient flow within the department.
  7. Accurately records the patient interview, history, and chief complaint in the patient record. Is responsible for accurate, concise and timely documentation of all assessments, interventions and patient outcomes in the medical record. Conduct chart audits on a regular basis to maintain compliance.
  8. Prepares equipment and assists health care providers during physical exams and procedures. Prepares and administers medications as prescribed and per physician orders. Recognizes and reports abnormal test results, changes in symptoms and the need for follow-up care.
  9. Assesses patient phone inquiries according to health care provider and departmental protocols and documents actions in the patients' medical records. Returns non-emergent telephone calls within 24 hours of receipt. Maintains communication with patients in regards to the status of inquiry or request.
  10. Retrieves and maintains medical records. Collects and files patient medical information into patient record such as transcribed medical reports, prescriptions, laboratory results, and any other relevant clinical correspondence. Responsibilities include chart assembly, analysis and discharge and release of the medical record.
  11. Maintains knowledge of reimbursement procedures; supports and follows through managed care and billing issues. Verifies prior authorization and appropriate consent forms are obtained prior to administration of services.
  12. Effectively communicates and collaborates with a diverse group of people for the purpose of informing the healthcare team and patient/family of plans/actions for teaching/education to benefit the patient/family and organization.  Effectively uses appropriate communication format in addressing professional issues.  
  1. Inspects equipment and facilities to ensure patient safety and proper functioning of equipment. Maintains communication with manager and other departments related to service needs such as necessary supplies, equipment (including repairs) and clinical training in order to effectively complete assigned duties.
  2. Provides care in a cost effective manner while promoting quality patient outcomes, monitors supplies and charges.
  3. Maintains strong customer service at all times. Identifies, prioritizes and responds to customer needs in a timely, professional and effective manner.
  4. Identifies and initiates own professional growth, actively pursuing professional development through continuing education and self-study. Responsible for attending education/in-service on current trends in case management, policies and procedures of the organization and changes in governmental, regulatory and accrediting agencies to ensure compliance with standards of practice.
  5. Identification of inter-department and intra-department performance improvement projects and carrying out of same.
  6. Perform other duties as assigned.

 

Qualifications

Minimum Skills, Knowledge & Abilities (including licensure, certification and other job-related credentials)

  1. Current state licensure as a Registered Nurse required.
  2. Graduate of an accredited RN nursing program. BSN or enrollment in BSN program preferred.
  3. Certified BLS completion required; for new hires, see CPR Requirement Guidelines for New Hires.
  4. Previous experience in a rehab setting preferred.
