Hours vary: Monday-Friday / 8AM - 5PM

 

 

Compensation depends upon candidate’s years of experience and internal equity.

PRN Rate: $46 per hr

 

Seeking an energetic and motivated PRN case manager/social worker to join our growing rehabilitation team in a busy Outpatient Day Neuro program. This program is focused on working with patients post neurological injury including BI, SCI, and CVA with an emphasis on return to work, school, or community activities. Case management duties include admission and evaluation of new patients, attending team and family conferences, discharge planning, equipment needs, and group education on community resources. This position is in Day Neuro with opportunity to assist in our Inpatient Rehabilitation hospital. Our hospital is located in Lakeway, Texas in the scenic hill country, a suburb of Austin.

Responsibilities

  • Serves as a primary point of contact to coordinate communication and collaboration within the transdisciplinary team, patient/family, central billing office and external stakeholders.
  • Monitors program delivery in collaboration with the transdisciplinary team to meet the patient expected outcome and provide resource assistance as needed.
  • Coordinates team and family conferences to review patient progress, problem solve barriers, receive family input and modify plans of care as needed.
  • Oversees insurance continued stay authorizations, confirming reports are meaningful and reflective of patient progress, affirms insurance benefits are available and documented in EHR for timely and accurate service billing.
  • Coordinates family training, oversees discharge planning, resource facilitation and communication of follow up services and appointments at the time of discharge.

Qualifications

  • Master’s Degree in Social Work required.
  • Current valid state licensure in related clinical discipline required.
  • Case Management Certification preferred.
  • Three years of previous case management experience in a neuro rehab setting preferred.
  • Certified BLS completion required.
