Program Manager / Physical Therapist

PT at Home Program

 

SSM Health Physical Therapy is seeking an experience Physical Therapist to lead and grow the outpatient PT at Home program.  In this position, you are responsible for treating your own caseload of patients, leading and growing the program and providing guidance than mentorship to the other therapists within the team.

Responsibilities

  • Facilitates staff development and mentorship through consultations, in services and development of human resources within the organization.
  • Helps foster competency through exceptional interviewing, orientation, and training of staff.  Develops and participates in core and program competency validation for staff; trains and educates as necessary.  Communicates all results with the CM/MM/VP.
  • Carries paatient caseload and provides comprehensive goal directed patient treatment to maiximize patient outcomes.  Organizes patient treatment in order to maiximize productivity and treatment efficiency on an individual and deparment basis.
  • Acts as a resource to the organization and the community in area of specialization through education, marketing, and promotion of services.
  • Identifies, responds and reports acute changes in patient status.  Follows appropriate emergency procedures to ensure patient safety.
  • Identifies and manages the various level of care necessary for optimal patient care delivery.
  • Promotes flexibility, team building and engagement in organization to promote an efficient and positive work environment.
  • Fosters interdisciplinary collaboration to ensure quality and efficient service delivery.  Effectively communicates to all members of the team in a timely, concise and professional manner.  Interacts positively with all personnel throughout the care continuum.
  • Works with other departments to develop and enhance the comprehensive scope of services.
  • Utilizes performance improvement methodology to improve quality of services.
  • Performs other duties as requested.

Qualifications

Minimum Qualifications

  • Bachelor’s Degree in Physical Therapy required.
  • Minimum of 2 years experience required.
  • Current and valid state licensure or eligibility for state licensure as a Physical Therapist required.
  • Cardio-Pulmonary Resuscitation (CPR) certification
