We are  currently seeking a Program Director to join our transdisicplinary team supporting the Neuro Transitional Rehabilitation Center.

 

Transitional rehabilitation is tailored to individuals with traumatic brain (TBI) or spinal cord injuries*, stroke or other neurological diseases as well as those who are at high risk for unsafe discharge.

 

Our Neuro Transitional Rehab is for patients that need a longer transition before they go home.  The facility is made up of suites where patients live full time as they learn to adapt to tasks, environments, and challenges that they will encounter daily.

 

 

Responsibilities

Job Summary

 

The Program Director is responsible for directing clinical program, day to day operations and referral development for Neuro Transitional Rehabilitation. Promotes clinical development, competency and expertise in staff to maximize patient outcomes, safety and quality. Effectively communicates on multiple levels throughout the continuum, program and eternally.

 

Job Duties

  1. Directs program and center operations to meet program quality and financial indicators.
  2. Facilitates staff development and mentor-ship through consultations, in services and development of human resources within the organization.
  3. Ensures/oversees staff competency through hiring, orientation and training of staff. Develops and participates in core and program competency validation for staff; trains and educates as necessary. 
  4. Establishes and modifies program delivery and operations as needed to promote service excellence for patients, referral sources and payers
  5. Ensures compliance to policies and procedures and program philosophy.
  6. Monitors and ensures compliance with all state licensing and regulatory guidelines and CARF accreditation standards.
  7. Act as a resource to the organization and the community to educate, market and promote neuro transitional specialty rehabilitation services. 
  8. Identifies, responds and reports acute changes in patient status. Follows appropriate emergency procedures to ensure patient safety.
  9. Ensures documentation for accuracy to maximize service reimbursement, manage claim denials and minimize any lost revenue.
  10. Works with the program Medical Director to maintain high quality of the program, including identification and assistance with research.
  11. Establishes, measures and reports quality metrics and patient outcomes associated with the program.
  12. Ensures the program philosophy is consistently incorporated into the program to include trans-disciplinary team approach, creating goals and functionally relevant activities to address patient impairments, primary therapist role, treatment floor model, community education and patient/family education model.  
  13. Promotes an exceptional employee experience through flexibility, team work, transparency,training to promote an efficient, effective and positive work environment. 
  14. Effectively communicates to all members of the team in a timely, concise and professional manner.  Interacts positively with internal and external stakeholders across the continuum and community. 
  15. Promotes the neuro transitional rehab program to all appropriate stakeholders (refers, payers, community).  Works collaboratively with the clinical liaison and communications department to develop targeted paper and digital collaterals as well as social media initiatives. 
  16. Pursues focused skill development through continuing education, self-study and involvement in professional activities and organizations. 
  17. Occasionally transports patients on community outings.

 

Qualifications

Minimum Education & Experience (Including Licenses)

  1. Current licensure in a clinical discipline, RN, Speech Therapist, Occupational Therapist, Physical Therapist, Neuropsychologist preferred.
  2. BLS certification through the American Heart Association required.
  3. Current State Driver's License in good standing required
  4. Knowledge of staffing patterns/ratios preferred
  5. Masters Degree + two years of leadership experience in an assisted living or rehabilitation environment preferred; experience with brain injury population preferred.
  6. Ability to delegate and problem solve effectively.
