Neuro Rehab Coach Supervisor

 

Full Time, Night Shift

Hours: 6:45p - 7:15a

 

Pay: $25.23 - $32.00 per hour

 

We are seeking a Neuro Rehab Coach Supervisor at our brand NEW OhioHealth Neuro Transitional Center. This is an exciting opportunity to change lives.  The Neuro Transitional Center has 12 beds and is designed to provide intensive rehabilitation to individuals with an acquired brain injury or spinal cord injury who are unable to safely return home at the time of discharge from an inpatient rehab hospital due to the severity of their impairments. Currently, 93% of our patients return to their homes at discharge from NTC, and now (Ohioans or Buckeyes) will not have to leave the state to receive this level of service.

Responsibilities

Supervises Neuro Rehabilitation Coaches (NRC), responsible for direct patient care and functions as part of the neuro transitional rehab team. Assists the NRC's with carrying out treatment plans and protects the wellbeing and safety of all patients; provides assistance to visitors and ensures that each shift runs smoothly. Serves as an educator of patients, families and coworkers.

 

 

On a daily basis:

 

  • Supervises staff on assigned shift, provides guidance as needed and communicates any staff concerns to Neuro Transitional Center Leadership (NTC), or RN/Administrator On Call.
  • Follow through and Supervises Neuro Rehab Coaches on patient plan of care in collaboration with other transitional team members including assisting in rehabilitation activities assigned to promote education, maintaining patient engagement. Assist and helps patients achieve appropriate goals in all environments including home, therapy and community.
  • Coordinates with NTC leadership to ensure appropriate staffing levels according to staffing guidelines and patient needs. Reports to NTC supervisor or Administrator on Call if there are any issues.
  • Assist patients in meal planning, food preparation, and assisting with grocery shopping. Prepares grocery and facility inventory. Carry out suite management and overall housekeeping tasks as assigned.
  • Responsible for coordinating patient transportation, and transporting as needed and ensuring transportation policies regarding vehicle maintenance, vehicle logs, and vehicle fueling are followed.
  • Attend scheduled trainings and/or meetings as needed.
  • Complete documentation and supervises NRCs to ensure required documentation according to Neuro Transitional Rehab (NTR) policy.
  • Follow through on all nurse delegation tasks as assigned including: vital signs, gastric tube management, diabetic management, O2 administration, crushing medications, intermittent catheterization of indwelling Foley catheters, catheter care, bowel and bladder program management and trach care
  • Responsible for medication administration for a max of 12 patients

Qualifications

Minimum Qualifications

  • Licensed Practical Nurse (LPN)
  • Current State Driver’s License in good standing required
  • Certified BLS completion required; for new hires, see CPR Requirement Guidelines for New Hires

Preferred Experience

  • Two years of experience in a residential care facility or rehabilitation environment preferred; experience with brain injury population preferred.
  • Medication aide experience
  • Ability to delegate and problem solve effectively.
