Case Manager Float - Full-Time

*Full-time (M-F 8am-4:30pm)*

 

Experience in physical medicine and rehabilitation strongly preferred.

 

 

SSM Health Day Institute is seeking a Case Manager to work in our specialized outpatient rehabilitation program and have the ability to float within the region to assist other Day Institute facilities.  We have nine centers across the St. Louis Metro area serving individuals recovering from a traumatic injury or illness who no longer require 24-hour nursing or acute rehabilitative care.  We take a comprehensive, multi-disciplinary approach to build skills and maximize independence.  If you like working in a dynamic, collaborative environment and are passionate about helping the neurological and medically complex patient, apply today to explore a new career opportunity with us!

Responsibilities

Under general direction monitors and assesses patient plan of care from pre-admission to post- discharge.

As part of the treatment team assists patient/family in goal setting to maximize patient’s independence in both home and community. Interacts with payers to obtain necessary authorizations for initiating and continuing care throughout admission to the program.

Qualifications

Professional Healthcare Degree in either 

  • Nursing Social Work 
  • Licensed PT, OT, SLP 

One year medical rehab case management experience (preferred)
