Responsibilities

We are looking for a full time PT to work on a multidisciplinary team that focuses on an integrated approach for intensive physical rehabilitation within Day Neuro Program. Program focuses on return to highest level of independence possible as well as community reintegration for patients with acquired brain injury. PT skill specific areas in this setting typically include: hemiparesis/hemiplegia, neuro re-ed, return to work, and community integration. Patients are treated as part of a multidisciplinary team, six hours a day, Monday through Friday. Looking for candidates with brain injury experience and who are creative, innovative and work well on a team.

  • Initial and ongoing systematic patient assessment
  • Timely and accurate documentation using Casamba-Web Based documentation/ Home Care Home Base / Epic / Therapy Source
  • Interpreting assessment/diagnostic data including medical notes and surgery reports
  • Ensuring medical orders are transcribed and processed accurately
  • Promoting continuous quality improvement
  • Instructing and counseling patients/families as needed

Qualifications

 

  • Current state licensure or eligibility for state licensure as a Physical Therapist required.
  • Certified BLS or completion in the first 90 days of employment required.
